Insurance

For those who would like to use their insurance coverage, our billing manager will bill your insurance company directly. You will be responsible for your co-pay or deductible paid at the time of your visit.  If you need an authorization from your primary care provider (doctor) or your managed care company, you will need to have that completed prior to your first visit.

For more information about the insurance programs accepted by our therapists, please visit each therapists insurance information under their separate pull down menus.  If you still have questions, please call Katie the MFTA office manager at 352-1668.
